Clinical Integration Specialist - Full-time - 80 hrs/pp; 7a-3:30p & 8a-8p - Day/Evening rotation

Responsible for concurrent review, of medical record documentation, as specified by St. Cloud Hospital Utilization Review Plan, for patients presenting to Emergency Trauma Center (ETC).  Reviews record for documentation completeness and compliance with patient level of care status. Responsibilities include facilitation of appropriate physician documentation of care to accurately reflect patient severity of illness and risk of mortality. Interacts with physicians and other members of the care team to provide guidance and recommendations for admission or referral to observation.  Assesses patients for present-on-admission (POA) conditions to ensure accurate documentation regarding hospital acquired conditions (HAC). Communicates the appropriate concurrent information to the case management team and Clinical Documentation Specialists.    This position supports the concepts and philosophies of nursing as well as sound business practices. Supports patient- and family-centered care through The Compass: St. Cloud Hospital’s Nursing Professional Practice Model to achieve the hospital mission of improving the health and quality of life that reflects the healing mission of Jesus. Implements and abides by Customer Service Standards for a superior patient experience.  Supports and implements safety practices as appropriate.

 

Job Requirements

EDUCATION

Graduate from accredited school of nursing required.  Baccalaureate or Masters in Nursing preferred.

EXPERIENCE

Minimum of 3 years Acute Care experience required. Basic computer skills required. Performance improvement, quality assurance related experience is required.  Utilization review, coding, or case management experience preferred.  Sufficient training will be provided in lieu of required experience.

SPECIAL REQUIREMENTS

Current Licensure as a registered nurse in the State of Minnesota required.

National Certification as Clinical Documentation Specialist preferred.

Score above 70 on CDS competency exam.
