Physical Therapist
Per Diem position covering the Port Huron, St.Clair, Sanilac, Sandusky areas.
Functions: Under the direct supervision of the Administrator, provides a variety of physical therapy services to clients in their place of residence in accordance with the physician’s authorized plan of care and currently approved professional therapy methods. Communicates effectively verbally & in writing. Assists the physician in assessing the patient’s level of functioning and development (and revision as needed) of the plan of care by performing appropriate tests for physical therapy evaluation. Completes all areas of documentation in the required timeframe to meet regulatory & agency guidelines.
Experience: Must have a license to practice as a physical therapist in the state of Michigan and provide documentation. Graduated from an approved physical therapy curriculum program. At least two years of appropriate experience as a physical therapist. At least one year of experience in a health care or rehabilitation facilty with demonstrated competency in the practice of physical therapy. Must have reliable transportation and provide agency a copy of an unrestricted driver’s license and proof of current auto insurance.
