Job Description
Physical Therapist
The role of the Physical Therapist is to supervise and participate in the application of a wide variety of physical therapy treatments to enhance rehabilitation of physical disabilities, injuries, and disease processes.
1. Perform comprehensive assessment utilizing the OASIS data set;
2. Provide physical therapy services as prescribed by a physician, which can be safely provided in the home and assisting the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional abilities test. 3. Assist the physician in evaluating disability, injury or disease process of individual patients, and in determining the patient’s level of function. 4. Interpret plans and establish and implement the proper treatment in accordance with the physician’s orders. 5. Contact the physician directly for patient reports and care as needed. 6. Submit appropriate documentation of the patient’s progress and course of care to the Agency and the physician. 7. Observe and record activities and findings in the clinical record and report to the physician the patient's reaction to treatment and any changes in patient's condition, or when there are deviations from the plan of care. 8. Instruct the patient and caregiver in care and use of physical therapy devices; 9. Instruct other health team personnel including, when appropriate, home health aides and caregivers in certain phases of physical therapy with which they may work with the patient; and Evaluate the home environment and makes appropriate recommendations. 10. Participate in staff development and Performance Improvement activities as requested. 11. Supervise Physical Therapy Assistants and Home Health Aides as required. The physical therapists shall be accessible at all times by two way communication, which enable the physical therapist to be readily available for consultation during the delivery of care. 12. Instruct the caregiver on the patient's total physical therapy program. 13. Maintain confidentiality of all patient and Agency matters. 14. Assist the physician in evaluating level of function, helps develop the plan of care (revising as necessary), prepares clinical and progress notes, advises and consults with the family and other agency personnel, and participates in in-service programs; 15. Act as Case Manager for Therapy only cases. 16. Attain quarterly goals as established in the annual performance evaluation. 17. Submit required documentation in a timely manner. 18. Travel may be required for business purposes. If so, the employee must have a valid driver’s license issued by the state in which they work and a satisfactory driving record.
Job Requirements
Qualifications: • Registered, licensed or certified by the state to practice Physical therapy with one to three (1-3) years as a physical therapist, homecare experience preferred. • Graduate of a physical therapy curriculum approved by: The American Physical Therapy Association, or • The Committee on Allied Health Education and Accreditation of the American Medical Association, or • The Council on Medical Education of the American Medical Association and the American Physical Therapy Association; or • Meets the requirements for membership in a member organization of the World Confederation for Physical Therapy. • Minimum of one year of clinical experience in the practice of physical therapy. • Must be able to lift the equivalent weight of an adult sized patient.
