THE OPPORTUNITY

The successful candidate will function as a member of a clinical team, will maintain patient flow and maximize the delivery of quality care to patients.  Candidate must emphasize health promotion and disease prevention through patient education and coordination of care.  

THE WORK

The RN Clinical Lead will provide primary and episodic care to patients – independently and in collaboration with other medical professionals – while following standards and protocols set by the sponsoring physician.  Establish and maintain effective working relationships with patients, their families and staff members.

KEY RESPONSIBILITIES

  • Identify and problem solve issues with appropriate services to ensure positive patient outcomes, utilizing cost efficient covered services
  • Provide an integrated holistic case management telephonic approach to those with chronic medical and behavioral health conditions with the goal of reducing the clinical gap between inpatient and outpatient services and potentially prevent avoidable hospitalizations and hospital re-admissions
  • Assist with discharge planning and care coordination to ensure optimal outcomes
  • Maintain accurate record of case management assessment and intervention including benefit and cost analysis for data collection and trending
  • Promote active involvement and initiative to members regarding their health care management and navigating health care delivery systems in order to preserve benefit resources
  • Communicate effectively with members, physicians, and providers; facilitate, advocate, and educate on the disease process; be a liaison and provide referrals to other departments and programs as needed
  • Be responsible for abiding by and supporting the care coordination program in order to ensure quality and efficient clinical operations

  • Job Requirements

    EDUCATION AND EXPERIENCE

    • College degree and valid Florida Registered Nurse (RN) license required
    • Minimum of 2 years medical and surgical clinical experience
    • Minimum of 1 year of case management, utilization review, or disease management
    • Strong experience with discharge planning
    • Excellent verbal and written communication skills, problem-solving, clinical assessment, care planning skills, and independent decision-making capability
    • Computer and organizational skills, ability to manage competing priorities, multi-task with results oriented outcomes and work in a fast paced environment
