Job Description

PRIMARY PURPOSE/FUNCTION:

Acts as a Case Management Consultant for client hospitals. Assists hospital clients with Case Management issues, clinical best practices, Risk Management / Performance Improvement, documentation, work flow and customer service.

                                                                               

 

PRIMARY RESPONSIBILITIES:

  • Serves as a Consultant and Liaison
  • Prepares new facilities for start-up / onboarding
  • Articulates / applies a working knowledge of Acute Care Hospital processes, including Utilization Management, Quality, Reimbursement, Length of Stay, Denial Management and Discharge Planning with Case Managers, Physicians, Clients
  • Acts as a consultant with current issues such as: utilization review / compliance, discharge planning, work flow, and adherence to current CMS regulations and accreditation standards related to Case Management
  • Serves as a resource for policy / procedure development
  • Travels to assigned facilities as needed
  • Assists with and prepares reports and correspondence, as requested
  • Maintains awareness in the categories of patient satisfaction, quality, risk, and core measures
  • Develops a high level understanding of Schumacher Group EM & HM programs
  • Assists with or conducts education for clients
  • Develops and maintains excellent working relationships with the facility’s Case Management Director
  • Addresses customer service issues as they occur
  • Collaborates / complements the Schumacher Group Nurse Liaison and Practice Coordinator roles, when Emergency Medicine or Hospital Medicine contracts exist at client hospitals

    Job Requirements

    QUALIFICATIONS:

    Educational and/or Professional Qualifications:

    • Registered Nurse – graduated from a qualified program or nursing school, possess an active RN license and in good standing with the State Board of Nursing
    • Case Management certification required

     

    Previous Experience:

    • Hospital Case Management Director experience required, 5 years minimum at the Director level
    • Knowledge and experience with clinical and Case Management best practice
    • Familiarity with CMS and JCAHO standards
    • Demonstrated customer service excellence

     

    Other Special Training/Skills:

    • Administrative and/or management experience
    • Ability to navigate through multiple systems
