Harrison’s Hope . . . A Caring Hospice is now accepting applications for a full time C.N.A. (Certified Nurse’s Assistant). We are seeking a detail oriented leader to add to our dynamic team. Job Description – C.N.A/Hospice Aid

POSITION SUMMARY

The C.N.A. will provide personal care services under the direction of the RN Case Manager and/or Lead Supervising RN. The C.N.A. is assigned to specific clients by the RN Case Manager or other appropriate professional and performs services for clients as necessary to maintain their personal comfort.

QUALIFICATIONS

  • Successful completion of a formal certification training program and/or a written skills test and competency evaluation.
  • Be at least eighteen (18) years of age.
  • Minimum of six (6) months work experience in a supervised setting, preferably health care facility
  • Demonstrated ability to read, write, and follow a written Plan of Care.
  • Good verbal, written, and interpersonal skills.
  • Licensed driver with automobile insured in accordance with Idaho and/or Harrison’s Hope requirements and in good repair.

    ESSENTIAL FUNCTIONS/AREAS OF ACCOUNTABILITY

    1. Performs comfort care and bathing as care planned under the direction and supervision of the RN Case Manager.
    2. Range of motion exercises if care planned
    3. Assistance in ambulation or exercises if care planned
      1. Performs personal care activities, including but not limited to:
      2. Bathing
      3. Shampooing
      4. Skin care/nail care
      5. Oral hygiene
      6. Shaving
      7. Dressing

     

    1. Performs household services essential to health care at home, including but not limited to:
    2. Meal preparation/feeding
    3. Laundry
    4. Light housekeeping
      1. Reports any observed or reported changes in the client's condition and/or needs to the RN Case Manager. Follows reporting guidelines in the Care Plan.
      2. Documents care provided and completes the forms required for the client's records. Completes the appropriate records to document care given and pertinent observations. Submits documentation daily per our requirements.
      3. Promotes personal safety and a safe environment for clients by observing infection control practices, following our guidelines, and reporting unsafe situations to your supervisor.
      4. Demonstrates safe practice in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs.
      5. Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.
      6. Maintains confidentiality in all aspects of the job.
      7. Attends in-service programs to meet compliance requirements. Identifies learning needs to your supervisor.
      8. Performs other related duties and responsibilities as deemed necessary.
