Job Description

Options Behavioral Health System provides inpatient psychiatric services to the Adult and Child/Adolescent population as well as services for Intellectually Disabled youth.  We are actively seeking a Discharge Planner/Casemanager.  This employee is responsible for coordinating continuum of care activities for assigned patients/residents and ensuring optimum utilization of resurces, service delivery, and compliance with external agencies and referral sources.  This employee is also responsible for setting appointments, completing biopsychosocials, treatment plans, contact with community referrals, and working in Acute and PHP programs.

Job Requirements
  • Bachelor degree in behavioral sciences or related field
  • Ability to assess a patient that can possibly hurt himself/herself and others.
  • Experience in psychiatric patient care.
  • Possess sufficient clinical background with specialization in clinical theories pertinent to ID/MR/DD-MI (and some sexual maladaptive behavior) children, adolescents, and residential treatment.  (preferred)
  • Strong interpersonal and communication skills, ability to work effectively with a wide range of constituencies.
  • Ability to plan, implement, and evaluate individual patient care programs.
  • Must possess valid Indiana driver’s license.     Minimum 21 years of age.
