Job Description POSITION SUMMARY

Responsible for gathering health assessment data to determine patient problems/needs; and establishing an individual interdisciplinary plan of care.  Measurable outcomes are incorporated through collaboration with the patient, family, significant others, and members of the health care team.

           
Nursing interventions and actions are consistent with the plan of care to restore, improve, promote health, and prevent complications and sequel of illness.  Evaluation of responses to interventions occur continually with revision of the plan of care in order to achieve desired outcomes.  Continued appropriate care is ensured through collaboration and coordination of community resources in discharge planning.

           

The RN participates in peer review to assure high quality nursing care.

Job Requirements

Education

Associate's Degree (two year college or technical school)

 

Experience

Six months to one year related experience


Certificates & Licenses

Current RN licensure by the appropriate state licensing board, driver's license, and automobile liability insurance.

 

Other Requirements

Knowledge and ability to effectively implement and document nursing theory, practice, and principles.  Good communication and interaction skills.
