We are looking for an experienced Registered Nurse Case Manager interested in relocating to the Houston area for this full-time position. There are many living possibilities, you could live in the city, the suburbs, or in the country east of Houston, and still be a short drive to the Hospital. There is a lot of new development going on near the Hospital which also has outstanding school districts located nearby. The RN Case Manager will comprehensively plan for case management of a targeted patient population. Holds joint accountability with social worker for facilitating psychosocial and continuing care. Works with the physicians and multidisciplinary care team to facilitate efficient, quality care and achievement of desired treatment outcomes. Communicates clinical information to payers to ensure reimbursement and will work closely with the business office.
