Job Description

Care and Services

  1. Accurately perform and record vital signs, height and weight.
  2. Accurately calculate and record weight gain/loss and fluid intake/output.
  3. Maintain a clean and safe resident environment.
  4. Recognize abnormal changes in body functioning and importance of reporting such changes to a supervisor.
  5. Perform palliative and post-mortem care.
  6. Perform routine care rounds on each assigned resident, at minimum, every two hours (or more frequently as indicated).
  7. Supervise, assist or provide ADL's such as:
  8. Bath, shower, whirlpool, and bed-bath
  9. Shampoo, grooming, shaving, nail care, and mouth care
  10. Dressing/undressing, application of splint/prosthetic devices
  11. Toileting, bowel/bladder training, incontinent care and peri-care
  12. Meals/snacks, hydration, proper use of feeding devices/techniques
  13. AM and PM care per facility policy
  14. Accurately calculate and record percentage of meal intake.
  15. Provide preventative skin care techniques, observe the resident's skin condition and report any changes to a licensed nurse.

Minimum Qualifications:

  • Freedom from illegal use of drugs.
  • Freedom from use of and effects of use of drugs and alcohol in the workplace.
  • Anyone found guilty by a court of law of abusing, neglecting or mistreating individuals in a health care related setting are ineligible for employment.
  • Must be at least 18 years of age.

Education and/or Experience: High school diploma or general education degree (GED), or three-six months related experience and/or training or equivalent combination of education and experience preferred.

Certificates, Licenses, Registrations: Graduate of a state approved nursing assistant training program and passed a state approved competency examination (written and skills) within 120 days of employment; name entered into state nurse aide registry.
