The Health Navigator will provide direct support to three general population shelters in Upper Manhattan and the Bronx. This role augments and supports the on-site shelter case management staff. The Navigator will work with Shelter staff to identify unmet needs among shelter residents help to develop outreach plans to engage identified shelter residents, engage shelter residents in supplementary services. He/she will accompany shelter clients to medical appointments, lab work and other medical tests. The Navigator will coordinate care with all medical providers and assist in having prescriptions filled. The Navigator will advocate on behalf of consumer regarding medical treatment as needed. This role is comprised of 50% field work.
