Job Description JOB SUMMARYThe Coding Specialist performs and facilitates complex coding: Reviews health records, identifies key clinical data elements within the record, and translates this data from verbal description of disease, injuries and procedures into numerical designations, applying ICD and CPT coding systems. He/she ensures coding compliance and acts as technical resource, resolves issues, trains and monitors new coding staff.He/she may be assigned to perform concurrent coding.PRIMARY RESPONSIBILITIES (ESSENTIAL FUNCTIONS)1. Reviews complex patient charts and assigns appropriate codes to diagnoses and procedures, includes but is not limited to:" Is responsible for reviewing and managing the most complex coding cases." Ensures coding compliance; applies all coding guidelines and principles as defined in the Coding Clinic, CPT Assistant, and leading authorities." For both inpatients and outpatients, reviews entire medical record and assigns the correct ICD and CPT codes to each data element" Complies with standardized coding standards and conventions and regulations, corporate compliance standards, and reimbursement policies." Records and sequences clinical data in correct order using national definitions of the Uniform Hospital Discharge Data Set (UHDDS)." Effectively uses applicable software applications to assign codes, determines a DRG/APC, and accurately enters codes into computer." Actively participates in DRG assurance program." Applies transfer rule for correct discharge disposition of records according to established policy." Maintains department best practice productivity and quality standards.2. Performs Concurrent Coding, if assigned includes but is not limited to:" Assists and performs Concurrent Coding on the hospital floors (for inpatients) and/or on outpatient records." Converts documentation into diagnosis codes and procedure codes." Identifies documentation opportunities in the concurrent medical record. Works closely with Care Management staff and clinicians to capture accurate documentation. Prompts physicians regarding missing, unclear, or conflicting medical record documentation. Assists in obtaining additional documentation as needed." Assists Care Management and HIM Coding staff in obtaining correct Transfer Rule DRG discharge disposition assignment." Assists in obtaining required Present on Admissions documentation." Reviews and codes accounts that need Concurrent Coding and DRG assignment.3. Investigates and resolves complex coding and billing issues, includes but is not limited to:" Acts as a technical resource; facilitates problem/issue resolution." Resolves PFS, Chargemaster accounts, and other billing issues." Contacts and works with physicians as needed for clarification of details of disease process or clarification of documentation to ensure correct coding." Expedites charts as necessary to obtain additional physician documentation." Discusses coding questions with team and Supervisor and reports unusual occurrences to Supervisor, Director of Health Information Management, or Compliance officer.4. Collaboration and Leadership, includes but is not limited to:" Stays current on all Medicare rules updates." Effectively trains and mentors new staff. Monitors new employees' work and gives input/feedback to management relative to employee performance." Covers for absences/vacations." Works professionally with all customers (MD's, departments, patients)." Makes suggestions and recommendations for improvements." Assists in performance of all quality initiative medical audits." Job RequirementsQUALIFICATIONS/REQUIREMENTS" Education Background1. Requires AA/AS in Medical Records.2. Or, comparable, equivalent work experience may be substituted for education with the approval of the HIM Director." Licensure, Registration, CertificationRequires two certifications/credentials, one from each of the following two categories:1) Either RHIA (Registered Health Information Administration) orRHIT (Registered Health Information Technician) from AHIMA - the American Health Information Management AssociationOr a bachelor year degree in a related medical field with HIM management approval.And2) Either CCS (Certified Coding Specialist) from AHIMA - the American Health Information Management Association or CPC (Certified Professional Coder) or CPC-H (Certified Professional Coder - Hospital) credential from AAPC - the American Academy of Professional Coders.NOTES:" Incumbents are required to maintain credentialed status through continued education credits." With the approval of the HIM Director, a bachelor degree in related medical field can be substituted for the RHIA or RHIT requirement." With the approval of the HIM Director, a Concurrent Coder with 10 years of experience (including at least 5 years acute care, major outpatient, utilization and concurrent coding experience) may be substituted for one of the above required certifications." Special Training/Skills" Typing: 25 wpm with accuracy required, verified via pre-employment testing." Position requires expert knowledge of disease processes, and ICD9 and CPT4 coding systems." Requires medical terminology." Requires knowledge of department, hospital and JCAHO guidelines." Demonstrated organizational skills, accuracy, and attention to detail." Ability to communicate and interact effectively with medical staff to resolve questions." Ability to train and mentor." Work Experience1. Requires a minimum of 2-5 years coding experience in an acute care or major outpatient setting.2. Extensive experience with applicable software applications and abstracting systems.3. For Concurrent Coder, utilization review and/or inpatient concurrent coding is preferred.
