Job Description

Job: Case Management

Responsible for promoting a collaborative practice among the healthcare team. Coordinates the integrated utilization management anddischarge planning program including monitoring, evaluating andimplementing steps to improve efficient quality patient care. Reviewsand evaluates admissions to assure each patient meets acute care criteria.

Job RequirementsA. Licensure/ Certification/ Registration: Current Utah Registered Nurse LicenseB. Education: ADN Required BSN preferred.C. Experience: Three to five years experience in an acute care setting. Two years experience in Case Management and/or Discharge Planning preferred.D. Special Qualifications: Excellent public relation skills, knowledge of community resources, knowledge of regualtory agencies.E. Degree of Supervision Required: General guidance and direction will be provided; however, must be able to function with minimal guidance.
