Job Description

The Home Care Liaison (HCL) is responsible for the overall census growth of the agency through the development, implementation, and evaluation of the agency marketing plan.

  • Daily travel from site to site per company needs
  • Participates in the development and implementation of the strategic marketing plan
  • Develops and maintains strong and effective working relationships with referral sources
  • Commitment and ability to meet goals for referrals, admissions, revenue, and expense control
  • Effectively and positively represents the services and programs of LCAH
  • Implements education programs designed to increase awareness of LCAH and generate referrals
  • Participates effectively in the coordination and transition of patients to home care services
  • Conferences effectively with external case managers and/or medical reviewers to clarify home care eligibility for non-Medicare patients
  • Performs strategic assessment of external environment
  • Orients new associates in their responsibilities in marketing the agency, including field rides and mentoring
  • Meets with or calls all patients prior to admission
  • Performs marketing analysis and establishes a sales strategy for business development with referral sources

  • Job RequirementsEDUCATION and EXPERIENCE

    • Bachelor's degree in Marketing, Business, or related field preferred; or LPN/RN with extensive case management and/or discharge planning experience; or PT/OT/ST
    • Familiarity with professional community preferred
    • One or more years of sales experience with proven results; prior experience in healthcare setting preferred

    SPECIFIC REQUIREMENTS

    • Excellent oral and written communication, interpersonal and presentation skills
    • Able to manage a territory with multiple sites and diverse customers
    • Knowledgeable of admissions and home care practices and procedures as well as the laws, regulations, and guidelines governing eligibility and admission requirements for home care
    • Self-motivated, goal-oriented, and able to work independently
    • Reliable transportation to travel per company needs
    • Availability for overnight travel to attend meetings, training sessions, and conferences as needed
