Complete DescriptionProvena St. Anne Center, located in Rockford, Illinois is a 179-bed, Medicare certified bed facility offering post-acute, rehabilitation and LTC nursing services in a resident choice environment. WOUND CARE NURSEQualifications: RN, with IL license in good standing, Wound Care Certification in advanced wound care through appropriate professional agency preferred or ability to obtain within six months of hire, (required). Educational scholarship is available for Wound Care Certification course. Two (2) years general wound care experience preferred, working knowledge of nursing regulations governing long-term care and rehabilitation, able to work with limited supervision and to provide supervision when needed, certification in CPR, ability to express or exchange spoken and/or written ideas in English with patient/residents, co-workers, families, and the public Essential Functions: Responsible for ensuring delivery of patient/resident wound care. Maintains adequate supply of materials for each patient/resident, collaborates with interdisciplinary health care team, patient/resident, and families in the prevention and/or treatment of skin care conditions, initiates, evaluates and revises plan of care for prevention of skin breakdown and treatment of skin conditions, wounds, and pressure ulcers, analyzes selected patient care plans for evidence of documentation to standards of practice, shares findings with staff, maintains and teaches others the specialized knowledge and skill necessary to perform techniques and routines of wound care, participates in ministry quality improvement program(s); collaborates in setting standards of care, monitors, collects, analyzes, and reports data related to skin and wound care management within ministry, serves as liaison for referrals to other specialty clinics and community resources, completes all required assessments and documentation within appropriate time lines, participates in development of policies, procedures, and systems to support prevention and management of skin conditions, wounds, and ulcers, conducts continuing education programs regarding wound management including but not limited to: Prevention; Early Identification/Risk Assessment; Causes of Skin Breakdown; Wound Assessment; Wound Management; and Documentation. Supports Administrative Nursing Team, as needed, assists nursing staff in daily duties as needed, participates in the on-call rotation of administrative nursing, works as floor nurse as needed. Performs other duties as directed by the DON/ADON.Provena Health is committed to diversity.
